Community Participation Support
Getting out, joining in, and belonging. We support people to shop, volunteer, take classes, visit the library, enjoy local events, and build friendships — with a trusted support worker by their side.
Horizon Bridge Supports LLC provides warm, one-to-one support for people with intellectual disabilities and autism — at home and out in the community — so each person can live with more independence, confidence, and joy.
What we do
Every plan starts with the person: their goals, their routines, their choices. Our direct support professionals walk alongside each individual to build real skills and real connections.
Getting out, joining in, and belonging. We support people to shop, volunteer, take classes, visit the library, enjoy local events, and build friendships — with a trusted support worker by their side.
Patient, respectful help with the rhythms of daily life at home — cooking, routines, personal goals, and staying safe and confident — always encouraging the person to do as much as they can, their way.
Friendly, reliable companionship for people who benefit from a steady presence at home — conversation, activities, supervision, and peace of mind for families.
Short-term relief that lets family caregivers rest and recharge, knowing their loved one is with someone patient, trained, and kind.
Step-by-step coaching toward each person's own goals — money basics, using transport, preparing for a job, and speaking up for themselves.
Getting started: Horizon Bridge Supports LLC is completing its enrollment with Pennsylvania's Office of Developmental Programs (ODP) in Delaware County and is already talking with families and supports coordinators ahead of launch. If you're planning ahead for support, we'd love to hear from you now.
How it works
Call or send the form below. Tell us about the person — their strengths, interests, and what a good day looks like.
We work with the individual, their family, and their supports coordinator to shape support around their goals and waiver plan.
A careful, personal match — the same familiar face, building trust visit after visit.
New skills, new places, new confidence. We check in with families regularly and adjust the plan as goals grow.
Leadership · Meet our founder
Founder & CEO, Horizon Bridge Supports LLC
Seun is a nurse and caregiver with more than eight years of experience supporting people with intellectual disabilities and autism — in family homes and community programs. Families who have worked with him know the same thing: he shows up, he listens, and he treats every person he supports with patience and respect.
He founded Horizon Bridge Supports so that the people he serves — and their families — could count on one trusted, familiar team: support that feels personal, led by someone who does this work himself, every day.
“The people I support are some of the most important people in my life. Helping them live a better life is what gives my work meaning.”

Questions
We serve Delaware County, Pennsylvania — including Drexel Hill, Upper Darby, Havertown, Media, and nearby communities — in people's own homes and out in the community.
Horizon Bridge Supports LLC was founded and is led by Seun Ogunmefun Akinsemoyin, our Founder & CEO — a nurse and caregiver with more than eight years of experience supporting people with intellectual disabilities and autism. You can read more about him in the Meet our founder section above.
Our services are designed for Pennsylvania's ODP waivers — the Consolidated, Community Living, and Person/Family Directed Support waivers — which fund home and community-based support for eligible individuals. If you're not sure about eligibility, your supports coordinator can help, and we're glad to talk it through as well.
That's the goal. Trust grows with familiar faces, so we match each person with a consistent support worker and keep families in the loop.
